VANCOUVER — A 26-year-old man with a history of violence has escaped for the second time from the Forensic Psychiatric Hospital in Port Coquitlam.

Matthew David Mikolajczyk’s most recent escape was on June 16.

The RCMP warns that Mikolajczyk is considered to be a risk to himself and the public. He he has a history of violent assaults.

A Canada-wide warrant has been issued for his arrest.

Mikolajczyk, who is six feet tall (160 cm) and weighs 170 pounds (77.1 kg), was last seen wearing a red T-shirt, white pants and had a full beard. He also has a seven-inch scar on his right arm.

He earlier escaped from the forensic psychiatric unit on Sept. 4, 2008.

About a week later, he was spotted by several residents on Saturna Island. Two Mounties went to the island and located the man but a struggle ensued – Mikilajczyk punched both officers in the face and fled into the bush.

The officers called for backup and the Victoria Police Department dog squad, an RCMP helicopter, an RCMP catamaran, members of the Sidney RCMP and the local fire chief joined in the four-hour search.

Mikolajczyk was eventually found on a steep embankment. He was arrested and returned to the Forensic Psychiatric Hospital, which holds patients who are found not criminally responsible for their crimes, including murder, because of a mental disorder or are found mentally unfit to stand trial.

Police advise not to approach Mikolajczyk but instead call 911.
